Apply for Replacement of Licence Card

Application Details

  • This form may take around 5 minutes to complete.
  • All fields are mandatory unless otherwise stated.
  • You will be asked to provide the following information:
    • Type of Licence for Replacement Card
    • Reasons for requesting Replacement Licence Card
  • You will be asked to provide the following documents:
    • Official document for change of Registered Name (E.g. Deed Poll)
    • Police Report for Lost Licence Card
    • A copy of the front and back of your current licence card (for damaged/defaced card)

Replacement Licence Card Fee

  • The replacement fees for Worker Licence Cards are as follows:
    • Electrical Worker: $21.40 (inclusive of GST)
    • Gas Service Worker: $21.80 (inclusive of GST)
    • Cable Detection Worker: $21.40 (inclusive of GST)
  • Accepted modes of payment:
    • PayNow
    • Credit/Debit card
  • Fees paid are non-refundable.
